Care represents our commitment to comprehensive health and wellness for all residents via our expert staff and on-site services such as PT/OT.
Connection encompasses all the ways we provide meaningful interactions for residents and their family from socialization to robust activities to in-room Vineyard Connect℠ technology.
Community includes all the unique features that set Vineyard apart, starting with a Life, Your Way lifestyle assessment that allows each resident to choose their desired programming, daily schedule and dining options.
The three of these integral facets combine to create the best possible experience for our residents as they transition into this new season of life.

Community Partners
Vineyard Johns Creek is excited to partner with Transitional Care Physicians of America. They will be a wonderful additional to our amazing team to offer a continuum of care to our residents.
Transitional Care Physicians of America will provide 24/7 availability and monthly community visits for primary care by Dr. Mohan or Hila Oren, N.P., along with:
- More frequent visit for acute needs
- Medication management/optimization and care planning
- Symptom and chronic pain management
- Depression/cognitive function/behavior management
- Home Health services coordination to achieve physical function optimization
- Transitional Care, Palliative Care, Geriatric care-focused dementia care
- On-site lab collection, and radiology services
- Discharge plan compliance
We are proud to announce our partnership with Powerback Rehabilitation who has proudly delivered 36+ years of trusted care for the older adult population. They offer a resident-centered rehab approach concentrating on sustainable recovery and for everyone to get their power back.
They will be providing physical, occupational and speech therapy for our residents. Physical therapy helps residents regain strength and balance, prevent falls, increase range in motion and decrease pain. Occupational therapy concentrates on safe, independent activities of daily living including cooking, bathing and dressing. Speech therapy focuses on communication problems, memory loss and swallowing dysfunction.
In addition, Powerback Rehab also offers these specialty programs:
- Age in Motion – healthy lifestyle living through wellness offerings.
- Balance in Action – evidence-based fall risk program with positive outcomes.
- Compass Program – designed to help residents lead a safe journey through the various stages of dementia.
- Everyday Safe Living at Home – services to help individuals safely remain in their home setting by making adaptions to their environment.
